For your convenience, new patient forms are available online.

You may pre-register with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

Please complete and submit the forms at least 48 hours in advance of your first appointment to accelerate the check-in process. Allow 1 to 2 days for insurance verification.

Absolutely THE best dental experience I have had in my entire life! I had been suffering from a failed root canal (done by a dentist I shall not name). Dr. Gannon and his staff took care of my needs (xrays, antibiotics, etc) until I was able to get the tooth surgically removed. No pain and aftercare follow-up with them was great! If there were 10 stars I would rate them at that! I wish they did regular dentistry there too.
